•Adult Health History (TP-10)
•Girl Health History(TP-25)
•Complete and return to Executive Director within 24 hrs. of any accident/illness going to, during, or leaving a Girl Scout activity.
•Wallet size card; keep with you at all times.

•Use if a Girl Scout is taken for treatment; parent/guardian and leader sign form, return to Service Center when completed.

•Completed by adults attending Girl Scout activities.

•Copy to TC and Membership Staff, troop telephone chair/emergency contact person.
•Keep as a permanent permission to attend regular troop meetings and for medical release.

•Keep as permanent health record for the year.

•Record skills/resources available to the troop.

•Follows the girls from troop to troop

•Use if you are collecting troop dues

•Girls can even maintain the records themselves

•Useful in tracking badge/activity completion
TROOP SPONSOR(S) •Troop/group Sponsorship Agreement (TP-15) (see council policy) •Complete & return to the Membership Dept. at the Service Center for pre-approval.

•Troop/Group/Service Unit Money Earning Application (TP-17)

•Complete & return to Membership Staff for approval 1 month prior to proposed activity
FORMS USED FOR PUBLIC RELATIONS
News Note (PR-1)

Sudden Service (PR-2)
•Used to report on troop, service unit or Juliette activities.
•Used for troops willing to be on call for special requests  of flag ceremonies and PR opportunities.
•Turn in completed forms to the Service Center
SPECIAL TROOP RECOGNITIONS •Target Troop Application (PG-14)
•Summer Action Troop Application (PG-15)
•Patches available to troops who meet specified requirements. Complete & return to the Program Dept. at the Service Center.
TRIPS, OVERNIGHTS, CAMPING AND SPECIAL ACTIVITIES •Troop Travel/Overnight/Special Activity form (TP-16)
•Troop Travel Evaluation (TP-16A)
•Check policies & procedures for training and other requirements for the types of activities.
